In order to better ascertain whether you are a possible candidate for facial plastic surgery, Dr. Haworth will need four of your facial images. For other personal plastic surgery concerns or questions use the form located here.
Using a camera on either an Android device or computer, orientate yourself according to the facial outline guides provided below. Press the camera icon associated with that particular orientation. If you are an Apple iOS user, you will need to upload a photo you have already taken on your device by using the “UPLOAD" button.
If you need to re-take your photos you may clear out your photo and try again by selecting "REJECT”.
Make sure you fill out all the form fields, including a short description of your concerns. Our office will contact you after Dr. Haworth has studied your initial photographs and feels you may be an appropriate candidate to undergo plastic surgery by him.
